If you wish to order by mail and pay by personal check or money order, please print and mail the completed form below with your payment.  Print information clearly.  Make personal checks and money orders payable and mail to:  Therese Skinner, 9808 NW 72nd Ter, Weatherby Lake, Missouri 64152-1816.  All checks and money orders must clear before order will be processed and shipped.  Returned checks and money orders will be charged a returned check fee of $35 payable immediately upon notice.  If you wish for your order to be mailed to Canada, please email first for shipping rates.
