    If you wish to order by mail and pay by personal check or money order, please print and mail the completed form below with your payment.  Print information clearly.  If paying by check:  name, address, city and State MUST be imprinted on the check.  No counter checks will be accepted.  Please email for postal mailing address to mail all orders.  All checks and money orders must clear (allow at least 7 to 10 days for checks to clear) before order will be processed and shipped.  Returned checks and money orders will be charged a returned check fee of $35 payable immediately upon notice.  If you wish for your order to be mailed to Canada, please see our FAQ's page first before ordering.
